Continuity Care Home Nurses is a full service licensed home health agency which provides nursing and other therapeutic services to patients of all ages in the comfort of their own homes. We will also provide support for other health professionals in the home through our multi-disciplinary approach- including physical therapists, occupational therapists, speech therapy, dietary consultation, medical social workers, and certified home health aides as ordered by the patient’s doctor. All of our employees are subject to background checks , drug screening, criminal background check and finger prints and we are licensed, bonded and insured.
